The Take Charge! Live Well! program is a new wellness and health promotion program provided in partnership with APS Healthcare. This program provides you with on-line access to valuable tools, information and services to assist you in making healthy lifestyle choices. The program is available at NO COST to state of Ohio employees, and their spouses, enrolled in a state of Ohio health plan. Download a copy of the program brochure, which provides complete details.

The state of Ohio wants to support you in living a healthy lifestyle. As a part of the Take Charge! Live Well! program, you can earn incentive awards in your paycheck by engaging in healthy activities and using the programs available on this site. Specifically:

Please note: When you register on the web site for the first time, you will be asked for your “Member ID.” This is your State 8-digit OAKS employee ID number.
The HRA is the first step to a healthier you and allows you to complete a comprehensive assessment of your overall health. You have the option to complete the HRA online or through the mail. To request a hardcopy version of the HRA, simply call 866-272-5507.
Once you complete the HRA, you will receive a personalized wellness plan, as well as ongoing education tailored to your unique situation that will guide you in making healthy changes for life. Complete your HRA and earn a $50 award in your paycheck!
Take Charge! Live Well! also provides you with access to personalized, lifestyle behavior change programs to address specific health concerns, including smoking, stress, nutrition and weight management. Each lifestyle program includes:
- A comprehensive, condition-specific assessment
- Personalized and tailored action plan
- Follow up communications, education and support timed to address critical points in your action plan
